How do I make a referral?
Referrals can be made to Connect Mental Health Services by voluntary and statutory agencies or by the service user themselves.
None of our services are direct access and we follow a detailed referral procedure which includes sending for references from other professionals before we can make a decision. However, once we receive the completed application form, we aim to complete referrals within three weeks from the date of the returned application.
For us to be able to consider a referral the following must apply:
-
The person needs to be aged 18-65 years, although exceptions will be considered;
-
The person must have a need for the support offered;
-
A risk assessment will be done to consider, particularly with regard to risk to the health and safety of the other tenants and staff.
Some house rules you may want to know:
-
There is a no smoking policy in the communal areas within the projects.
-
There is a 'zero tolerance to drugs' policy.
-
Alcohol is restricted to individuals' rooms only.
